This privacy policy describes how medical information about you may be used and disclosed and how you can get access to this information.

This page explains how United Dialysis Center handles information collected on this website and how that differs from information protected in treatment settings. For the full legal details about patient privacy under HIPAA and 42 CFR Part 2, download the: COMBINED NOTICE OF PRIVACY PRACTICES.
United Dialysis Center does not discriminate based on race, color, religion, national origin, ancestry, age, disability, sex, gender identity, sexual orientation, marital or family status, military service, political beliefs, or parental status, and provides free language and accessibility support upon request. For full details, see the COMBINED NOTICE OF PRIVACY PRACTICES.
United Dialysis Center accepts most major insurance plans, including Medicaid. Coverage and benefits vary by plan. Verification does not guarantee payment. Please contact our admissions team for individualized benefit information.
As a Medicaid provider, United Dialysis Center does not engage in balance billing for covered services. Patients will not be billed for amounts beyond state-approved copayments or coinsurance.
If you are uninsured or prefer to self-pay, you have the right to receive a Good Faith Estimate of expected charges before receiving services. For certain out-of-network emergency services and ancillary services at in-network facilities, federal law limits surprise billing and balance billing. For details or to request a Good Faith Estimate, contact our Billing team at [email protected].
The sections below apply to data collected when you browse this site or submit general inquiry forms. They do not replace your healthcare privacy rights. Patient information created or maintained in treatment is protected by the Combined Notice linked above.
